Nurse Care Manager (Palm Beach County)

Advocare Care Advocates is the preeminent private care management company in South Florida. We are based in Delray Beach and service Palm Beach, Broward, and Miami-Dade Counties. This position offers the right candidate to be home office based with a flexible schedule, starting part-time hourly with potential to gradually increase to a full case load.
Responsibilities and requirements include:
Initial client assessments to evaluate medical history and current status. Consultation with families of senior or disabled clients. Advocacy at home, hospital, or physician appointments. Management of medications and complex care. Coordination with other allied health professionals. Ability to be on call and cell phone responsive to client calls. Strong writing skills, capability to succinctly document all activities with complete nursing notes in secure online charting system. Ability to create professional care plan and concise communication. Case Management Certification such as CCM or CMC is a strong plus.
This position allows complete freedom and flexibility to work from a home office, schedule all your own appointments, and work very independently while earning great hourly compensation, with opportunity for growth for the right candidate. Position requires on-call availability to clients for occasional weekend and evening response, which is paid generously at time and a half.
In order to be an effective nurse advocate, you should have 10+ years of previous experience in community and/or facility based practice. Solid computer skills required. You must be a strong, diplomatic verbal communicator who also possesses excellent written communication skills. Currently we are seeking candidates who reside in and can service South Palm Beach County, Central Palm Beach County, and North Palm Beach County.

News from The Commission for Case Manager Certification. Changes in Continuing Education Requirements for Certified Case Manager (CCM).
The Commision for Case Manaer Certificaiton recently notifed all Certified Case Managers at the end of 2015 that at the time of renewal, they will need to include ethics among their continuing education requirments. CCMC states that starting with the 2016 renewal cycle, those case managers whose certification expires in 2016 will need to have 2 ethics CEs at the time of renewal. Those expiring in 2017, will need 4 Ethics Credits for renewal and those expiring 2018 and forward will need a total of 8 continuing education credits at the time of renewal. Per the Commission, the additional requirment does not increase the total number of continuing education credits you need to have, but the 2 ethics credits will be part of the 80 contact hours.
The Board of Directors of the South Florida Case Management Network has committed to adding ethics training as part of our efforts to provide continuing education credits for all certified case management members so they can meet the requirments set out by the Commission for Case Manager Certification as well as other Certififying Bodies.
